The Children with Special Health Needs Care Coordination Program is also called Children’s Medical Services (CMS) Program. It assists low income, uninsured families to obtain special medical care for their children with chronic medical conditions or disabilities by linking them to needed services. The Program works with families to coordinate care for their child's special medical needs. The Program can cover many types of services including: • Visits to a specialist • Hospitalizations • Surgeries • Medications • Therapies • Lab Work • X-Rays • Medical Equipment
