Led by health educators/doctors, AAHP education classes offer information, strategies, and practical skill-building to Montgomery County residents who want to prevent or manage diabetes. The 4-class series are accredited by the American Association of Diabetes Educators. For convenience, classes are held at various locations throughout the County.
An AAHP Certified Diabetes Educator (CDE) provides one-on-one self-management counseling sessions with clients. During an initial consultation, the CDE discusses with the client his/her needs, knowledge about diabetes, and current treatment plan. The client and CDE then develop, implement, and evaluate an individualized plan of care.
