JSSA Children and Family Services:Care Coordination

JSSA staff work with individuals with special needs, seniors and their families on an ongoing basis to assess specific needs and create personalized short and long-term care solutions. Care coordination is an approach to healthcare in which all of a client’s needs (most often for individuals with individuals with special needs or seniors) are coordinated by one point person. This, a JSSA social worker, who works with his/her client and the client’s caregivers (guardians, parents, adult children, etc.) to ensure that the client receives the most appropriate treatment – both within JSSA and through community resources. Our care managers help identify particular areas of need. They chart the right course of action and monitor and coordinate individualized care plans with their clients, family members and other professionals. Care managers provide access to a broad range of counseling, homecare, educational, social, and transportation services as well as advocacy and future planning needs. They are also very knowledgeable about community referrals and how best to work with local agencies and organizations.

Eligibility: For children and adolescents, ages 4-17 who are having trouble at school, seem unhappy or bored, exhibit problematic behavior, or are having difficulty developing and maintaining relationships.
