Case management services for street homeless adults in Montgomery County. Services include outreach, engagement, needs and housing assessments, entitlements and beneficiary referrals, advocacy, education, and connection to resources with a goal of connecting clients to shelter or permanent housing options.
•Clients must be age 18 and older, without an accompanied minor, actively living on the streets, unsheltered, and not connected to other Homeless Continuum of Care providers
•Clients can self-refer, be referred by the community, or by other county providers
•Clients can also be engaged on the street during regular outreach by case managers
•For immediate shelter options, please contact the Montgomery County Crisis Center at 240.777.4000
We welcome input, tips and referrals from community members regarding vulnerable, possibly unsheltered individuals who may benefit from our services and support. .
